no fine no rules,no body having time to punich you and you enjoy your tour/travel and the patient (normally heart patients,pregnant ladies) suffer.Generally only problamatic patients are these only,emergency no body knows when it wl come, but you can travel without attending as it is not binding on rly to provide doctor to the patients boarding on train, it is only an help to their customers by railways.which doctor waste his time in attending whole journey in attending patients for mere 10% concession.(rly also knows) that is why they write to help in case of emergency only
