2) Question might arise, if it is the same train why "Released" and why not use the same ticket..?
HO usually applies from start-end. In some trains however, there might exist specific quota for intermediate stations (Just like Tatkal) where you can either book from source station and change boarding point or book against the quota allotted to the intermediate station
Likewise, if you are boarding from an intermediate station which does not have a HO Quota of its own, the same needs to be "RELEASED" and "FORWARDED" from the source station/division to the concerned intermediate/charting station.
I have however seen exceptions when an intermediate station belongs to the same division and/or are pretty close by but still the PNR's are "RELEASED".

3) Fraudulent bookings using unscrupulous means.
Ex:- Fake data in IRCTC Profile/Using Agent ID during peak hours
When tickets are booked in unauthorized way say wrong passenger details or by using travel agent ID for booking between 8-12 or say I put up wrong information in my IRCTC profile the ticket will be forfeited. In such case, my booking is forfeited and the berth is immediately pushed back into the system instead of being cancelled as cancelling it will need to hold the berth till chart preparation while in this case it is directly released back into the system. (Opposite to what happens for #lowerberthquota and #ladiesquota)
