Case 1: The same loco is detached and taken onto the other side of the train and attached over there.

Case 2: Some other loco attached at one end and first loco detached at the other. This is very rare though. Mostly the same loco will be attached at the other end.

Loco Reversal: Loco is detached, taken through the loop line, and attached to the rear side of the train.
Rake Reversal: Loco in the front gets detached and another loco from the back gets attached at the end.

When the train enters a certain dead end station, the loco gets detached from the front n takes a loop, goes back n then gets attached back to the train. this is called Loco/Rake reversal. this is the original process which was followed before.
Now

In case of some terminal stations where the line ends, there are exit lines provided so that the loco doesn't get stuck up over there. The same is detached and makes a quick exit.

The funda is the same, as the Train changes direction. The train is switched onto some other line. So the train exits the platform in the same direction from where it entered. Only difference in rake reversal is loco change.

Not necessarily train would switch line. In dead stations like HWH, the loco simply gets detached from the front - another loco gets attached at the back - train proceeds to NE India.

The Loco detached from front & attached to rear (as the train needs to run in reversed/opposite direction from that station onward)!!
In some case, the loco detached & loco attached @ rear may differ (not same), in case like Diesel to Electric (& vice-versa) or to minimize TAT (turn around time)
