The Indore-Patna Express derailed early on Sunday because the wheels of a coach had jammed, officials indicated, as passengers complained they alerted authorities of an unusually loud sound but were asked to not worry.
Eyewitnesses of the accident – which killed more than 100 people – told HT that they told the ticket collector about a thunder-like sound from underneath their S1 coach in Jhansi, hours before the accident.

concern with the TTE at Jhansi. He told us it was normal,” said Deepak Rajbhar, 32, a resident of UP’s Azamgarh who was in S1 coach with his uncle Jai Prakash. He is currently admitted at a hospital in Kanpur.
He said other passengers told the guard that the sound was very loud and people in the coach could not hear each other properly.
Neeta Gujarati, another survivor from the S1 coach who underwent a CT scan at Lala Lajpat Rai hospital in Kanpur for head injury, said she was sleeping. “There was a jerk and I fell,” she said.
Railway officials, who did not wish to be named, said the problem appeared to be in the S1 and S2 coaches – the worst-hit by the accident – and it concerned the wheels.
“The wheel apparently became immovable. As a result the coaches derailed one by one,” they said.
“The way the coaches have derailed on the side and climbed onto each other points to this theory. The railways have begun a preliminary inquiry into the accident and the picture will be clear after that,” they added.

This is surely rail fracture case which is quite normal because in desert areas there is diff in temp during day night especially in the onset of winter..regular patrolling is necessary to prevent mishaps during such case but NCR is only busy running numerous trains alomgwith freights..

If this is the case then, who so ever was supposed to do maintenance should be held accountable and case of Manslaughter and Homicide should be filed, because of their negligence 100+ people died. Accountability has to be there.
I hope Railways fix this issue and update their maintenance, after all they charge us big bucks toooo.

(1) Actual reason can be known only after the detailed investigation by experts is over.
(2) Current temperatures are not too low our rails are working well at sub zero temperatures in Kashmir Valley.
(3) The age of the rail need to be taken into account
(4) The impact of the damaged wheel or the derailment itself may cause rail to break.

(5) After derailment many things rail, sleepers etc may be found broken, but what was the status before derailment can be judged by detailed investigation of the last 1 or 2 km of track crossed by the train before derailment and the inspection of vital coaches and its components like bogeys etc.
So many possibilities are there
(A) At first instance, politicians have tendency to point fingers at opponents, IR officials on the equipments supplied by the other organisations and Media/general public on RM/IR employees. Let the final report of investigation come out to know the real reason.
(B) Political expediency, financial imprudence, inherent inefficiencies in working/decision making and poor change management practiced over decades is the root cause for most of the maladies in IR today.
(C) Any body may be found responsible for this accident, but not much can be done to ensure safety unless the overall system improvement is done.
