Final report of 12506 accident at Raghunathpur (RPR).Track fatigue and failure of DNR division Engineering department to detect the failure main reason. UFSD report of track testing two months ago at the accident site is also present.Also 30521 GZB WAP 7 had some wheel defect issue related to dia which was not properly attended by GZB loco shed. Source-Rail whisper
